What it is for

Medical is the school's health record for the children in its care. The nurse keeps each child's conditions, the plans staff follow in an emergency, the medications a child takes and the doses given, the visits to sick bay and the immunisation register. Staff who care for the child read only what they need to act on, and the clinical detail stays with the nurse.

Health is a separate record from wellbeing. A nurse keeps no counselling note, and a counsellor keeps no medical record. Most of the work happens on a child's own page, in the "Health" and "Action plans" sections. The Medical area of the console reads across the school.

Key ideas

Condition
A health condition of a child, such as asthma or an allergy, with its severity, what it is, what sets it off and the dates it applies from and to. The school chooses which kinds it records.
Notice for staff
The short line the nurse writes so that each member of staff knows what to watch for. It shows beside the child's name on rolls and lists.
Restriction
A record that a child is not permitted to do an activity such as sport, swimming or camp, with the reason and the dates, until it is cleared.
Action plan
An anaphylaxis, asthma or other emergency plan for a child, with its document and a review date. A plan counts as due a number of days before its review date, which the school sets.
Medication
A medicine a child takes at school, with its dose and the authority to give it. Each dose given is logged with who gave it, when and who witnessed it.
Visit and case
A visit is one trip to sick bay. A case follows an injury or illness over time, with notes, follow-ups and messages to the family.
Immunisation status
Whether a child's immunisation is up to date, taken from the Australian Immunisation Register statement the registrar captures at enrolment.

On a child's own page, the "Health" section has their conditions and restrictions, and "Action plans" has their plans. A nurse also sees a "Health centre today" panel on the home page with the day's doses, sick bay and plans.

The Loans screen is the health centre's own register of crutches, wheelchairs and other items it lends. It lists what is out and to whom, marks a loan overdue against the due date given when it was lent, and names each item the health centre may still offer.

The Spare pens screen lists the school's spare adrenaline pens, soonest to expire first. A pen is marked "Expires soon" from a month before its expiry date and "Expired" from the day after, and it is on the morning findings from the same day. Press "Remove" once a pen has been used, thrown away or replaced. A parent's consent for a spare pen is recorded in the "Consents" section of the student's page, and the students with one are listed under the pens.

Recording a condition

  1. Open the child's page and choose the "Health" section.
  2. Press "Record a condition" and choose the "Kind" and the "Severity".
  3. Fill "What it is" and "What sets it off", then "Diagnosed on" and "Told by".
  4. Write the "Notice for staff": one line that tells a teacher what to watch for. It shows beside the child's name.
  5. Save it. To change a condition later, press "Edit". To end it, press "Close" and give the date.

Press "Record a restriction" to record that a child is not permitted to do an activity, with the reason and the dates. Press "Clear" when they are cleared for it again. A history stays on the page, so what was recorded on a given day can be read again.

Action plans, medications and doses

On the child's "Action plans" section, press "Record a plan" and choose what it covers, its kind, when it was issued, who issued it and when it is due for review. A replacement closes the earlier plan, and the plan's document is attached for the adults who supervise the child. The Action plans screen lists plans that are due and conditions still without one.

A medication is recorded against the child with its dose and the authority to give it. On the Medications screen, a due dose has a "Give" button. Choose the medication, say when it was given, why, and who witnessed it, then press "Add to the log". A wrong entry is corrected with "Add the correction", and the original stays in the log.

Sick bay, immunisation and the camp pack

On the Sick bay screen, press "Record a visit" and write what the child presented with and the treatment. Mark the visit serious where it is, and press "Record the departure" when the child leaves, saying how it ended. A visit can send a message to the family. Press "Open a case" to follow an injury or illness over time, adding notes and follow-ups, and closing the case when it is done.

The registrar records each child's immunisation statement and status at enrolment. The Immunisation screen is the nurse's and registrar's list of students whose status is unknown or not up to date, which can be filtered by year level.

For an excursion, open the Medical pack from the trip's page. It is named for the kind of trip, so a camp's is the Camp pack and a day excursion's the Excursion pack. It lists each student on the roster with their alerts, medications, action plans and whether a medical treatment consent is in force, and can be printed. A teacher with the plan capability can read it.

On the Loans screen, press "Add an item" to put crutches, a wheelchair or another item in the register. Press "Lend an item" to choose the item, who it goes to and the date it is due back. The row for an open loan has a "Return" button, and a retired item can be offered again.

What families do

Once a year, on the day the school sets, each family is sent a medical confirmation to check the details kept for their child. The family reads and corrects them in the portal under "Medical details". The nurse then approves or declines what the family sent, and an approval writes each changed fact to the child's page.

Training notes

  • Nurses need the whole chapter, about 45 minutes, practised on the baseline school.
  • Teachers and heads of house need only the notice beside a name and how to read an action plan, about 10 minutes.
  • Registrars need the immunisation record at enrolment, about 5 minutes.
  • Never train on a real child's record.
